N.Y. Hospitals Partner With Nonprofits to Reduce Admissions

Hospitals and social-service charities in New York are teaming up to cut down on costly hospital stays in low-income communities as part of a state Medicaid-reform program, reports Crain’s New York Business.

As part of the five-year, $7.4-billion initiative, hospitals can get Medicaid reimbursements for housing, financial, nutritional, and other aid aimed at improving overall health in their communities and achieve a targeted 25-percent reduction in avoidable hospitalizations.

Medical centers and managed-care firms are partnering with charities on a variety of programs and services, such as persuading local shops to stock healthier food and courses to help asthma and prediabetes patients better manage their conditions.
