A Template for a Planned-Gift Letter of Intent

Charities often ask donors who have made a revocable estate gift to complete a form to provide more information about the type of gift, its amount, and how they would like to be acknowledged.

While donors are not required to sign these documents, the information may be useful for the charity’s budgeting and planning.

This template from the Partnership for Philanthropic Planning will give you a sense of what type of information to request. It should be adapted to include only the types of gifts your organization accepts.
